In accordance with the Right to Information Act, No. 12 of 2016.
Requests for information should be made to the Information Officer of the Provincial Department of Health Services by completing and handing over a request, preferably in the manner prescribed in Form RTI 01 – although using the form is not mandatory. A request may be made in writing or verbally.
Send your request to the Information Officer. Appeals are made to the Designated Officer.
Submit your request for information (Form RTI 01 or letter) to this officer.
Appeals against decisions of the Information Officer are made to this officer within 14 days.
The Right to Information Act sets clear time limits for every step.
When you make a request for information, either verbally or in writing, you will receive an acknowledgement from the Information Officer.
The decision whether to grant the information or not will be given as expeditiously as possible, and in any case within 14 days.
If the information is to be provided, you will be informed of any fee under the schedule prescribed by the RTI Commission. Information is provided within 14 days of payment, or within 14 days of the decision if no fee is required.
If the information cannot be provided within 14 days after payment, the period may be extended up to a maximum of 21 days, and you will be given the reasons for the extension.
You may appeal to the Designated Officer in the situations listed below. An appeal must be made within 14 days.
Where a request concerns the life and personal liberty of a citizen, the Information Officer will respond within 48 hours.
An appeal to the Designated Officer must be made within 14 days. If you are still not satisfied, you may appeal to the Right to Information Commission.
